Assist-Life Stages, Transition and Support is a specialized service that provides a “safety net” during times of significant change. Whether you are moving from school to the workforce, transitioning into a new home, or managing a change in your health status, we provide the strategic planning to ensure you don’t fall through the cracks.

Proactive Planning and Mentoring

We offer short-term and long-term support focused on:

  • Coordination of Supports: Bringing all your service providers together to ensure a unified approach during a move or life change.

  • Capacity Building for Transitions: Training you on what to expect in a new environment, such as a new workplace or a university campus.

  • Crisis Intervention: Providing immediate support and advocacy if a current living or social situation breaks down.
